Target group:
Patients who have left from a care hospital or long-term treatment departments and also from other hospitals and do not need active treatment but nursing care.
Patients for whom the nursing care enables to prevent hospital treatment.
Patients with mobility disability.
Chronic in-bed patients with serious physical problems.
Patients with reduced mobility.
Patients who require complex nursing care that the social welfare employee or family members cannot provide.
Terminal patients and dying persons.

1. Select the type of applying:

To receive the service it is necessary to forward the referral to the office of OÜ Koduõde. The family physician or medical specialist sends the referral concerning the need for the service by e-mail to koduode@medicum.ee. After the arrival of the referral, the senior nurses contact the referral doctor and the patient.

Result

Presumable improvement of the quality of life of a person with a cronical illness.

What to do when the result is unsatisfactory:

Contact the service provider's managing employee.

To receive the service the referral has to be forwarded to the office of OÜ Koduõde and these are the following ways to do this:1. The family physician or medical specialist sends the referral about the need for the service, either by post, e-mail or fax. After the arrival of the referral, the senior nurses contact the referral doctor and the patient. 2. The referral can be forwarded by persons close to the patient whether personally, by post, e-mail or fax.3. Necessary information is available over the phone. Call to 6505092 during workdays from 9.00 to 16.00. In the Iru Care Home at the request of the doctor for service provision.
