In order to receive the service, submit a request by e-mailing [email protected] or on paper via post to the Tallinn Social Welfare and Health Care Department at Paldiski mnt 48a, 10614 Tallinn, providing the following information:

• applicant’s name
• applicant’s identification code
• applicant’s phone number
• e-mail address to which we will send a copy of the letter of guarantee (if you do not have an e-mail address, please include the address to which we can send a paper copy)
• the name of the service (rehabilitation, dental care or reimbursement of glasses)
• the name of the service provider (any service provider with the relevant license for rehabilitation; Tallinna Hambapolikliinik or OÜ ABCdent and Tallinna Optika OÜ for glasses)
• the desired time of use of the service (noting this is not mandatory or you can simply say at the earliest opportunity)

The letter of guarantee will be issued within two weeks of submitting the request (or further information); we will do our best to do this more quickly for time-sensitive matters. The letter of guarantee will be sent via e-mail to the service provider and a copy will be provided to the veteran. Tallinn makes the payment for the service directly to the service provider, not to the veteran using the service.
